We can only accept documents from which ALL personal identifiers have been removed or blacked out.
Please remove or completely cover the following 18 items before sending medical files to us:
- Names (patient, family members, anyone mentioned)
- Street address, city, county, precinct, or any geographic unit smaller than a state
- Full ZIP code (the first 3 digits are usually OK if the area has >20,000 people)
- All elements of dates (except year) related to the patient: exact birth date, admission date, discharge, procedure, or death date
- Ages over 89 (may be grouped as “90+” or “elderly”)
- Telephone numbers
- Fax numbers
- Email addresses
- Social Security number (full or partial)
- Medical record numbers (MRN)
- Health plan or insurance ID numbers
- Account numbers
- Certificate or license numbers (driver’s license, passport, etc.)
- Vehicle identifiers and license plate numbers
- Device identifiers or serial numbers (pacemakers, implants, etc.)
- Web URLs and IP addresses
- Biometric identifiers (fingerprints, voice prints, retina scans)
- Full-face photos or any comparable images
If any of these 18 items are still visible, we will immediately quarantine the file without opening it, notify you, and securely delete it within 5 business days.
